About EPCP
Cleveland Health Center developed the Enhanced Primary Care Program (EPCP) to support patients who benefit from closer monitoring, proactive care coordination, and more personalized follow-up.
EPCP is designed to identify medical concerns earlier, strengthen continuity of care, and reduce avoidable emergency room visits and hospitalizations through structured planning and ongoing clinical support.
EPCP adds structured care planning, proactive check-ins, and enhanced coordination between visits — complementing your regular primary care, not replacing it.
Program Overview
What is the Enhanced Primary Care Program (EPCP)?
EPCP is a structured care program designed for patients with complex or chronic medical conditions who benefit from closer follow-up, proactive monitoring, and coordinated care across providers and settings.
Who is EPCP designed for?
EPCP is especially helpful for patients managing multiple chronic conditions, frequent medication changes, functional decline, or increased risk of emergency room visits or hospitalization.
What conditions are commonly supported through EPCP?
EPCP commonly supports patients with heart disease or heart failure, diabetes, chronic lung disease, kidney disease, neurologic conditions (including dementia), and other medically complex situations requiring close coordination.
How is EPCP different from routine primary care?
While routine primary care focuses on scheduled visits, EPCP adds structured care planning, proactive check-ins, closer monitoring, and enhanced coordination between visits.
What services may be included in EPCP?
Depending on individual needs, EPCP may include personalized care plans, medication review, proactive outreach, coordination with specialists and hospitals, support during care transitions, and caregiver communication when appropriate.
Does EPCP replace my regular primary care visits?
No. EPCP is designed to complement routine primary care visits, not replace them. You will continue to see your provider as usual.
How does EPCP help reduce hospitalizations?
By identifying changes in health earlier, improving communication, and coordinating care across providers, EPCP helps address issues before they escalate into emergencies.
Are caregivers involved in EPCP?
When appropriate, caregivers may be included in communication and care planning to support safety, understanding, and continuity.
How do I enroll in EPCP?
Enrollment is based on medical appropriateness and eligibility. Our team will review your health history and discuss whether EPCP is a good fit for your needs.
Is EPCP covered by insurance or Medicare?
Coverage depends on individual eligibility and program structure. Our team will review coverage details and answer questions before enrollment.
